How to refer a patient

• Patient data: Name. e-mail, Identity, CPF, date of birth, telephone1, telephone 2, agreement

Information about patients

We need to receive all available medical records prior to the first appointment to ensure the best possible care and evaluation.

• pathology reports
• Operating Procedures / Notes
• Relevant diagnosis
• Image reports (Chest X-rays / CAT / MRI / Bone Scan)
• List of prescription drugs

It is important that the patient is aware of their indication and that they expressly authorize the sending of their medical documents to the professionals of the Oncoclínicas Group. Medical records are owned by the patient and are subject to professional secrecy.
